Floyd Foundation - Grant Request Form

The Floyd Foundation, managed by Central Trust Company encourages your organization to apply for a grant to continuing efforts to make Southwest Missouri a better place for all its citizens. We suggest gathering all of your documentation before completing the application, but you are welcome to save and come back to the form by clicking the “Save and Continue Later” link at the bottom of the form.
